Urinary Problems
(Patient Care Visit > Urinary > Urinary Problems)
This is where in the CHAM you would go to take a history of a urinary system problem.

Urinary 1: Bladder Infection
The following is prevention Patient Education for which urinary Assessment: "Urinate as soon as you feel the need. Do not hold urine in your bladder for a long time. Try to drink 6-8 glasses of water or other liquid every day. For women: Wipe from front to back after urinating or having a bowel movement."

Bladder Infection (Urinary 1)
You would make this CHAM Assessment for a patient presenting with need to urinate often, pain with urination, no fever, no CVA tenderness, and mild tenderness above the pubic bone.

Report ALL patients.
If you do not have a Standing Order for an Assessment you must do this.

Kidneys
These filter waste and extra fluid from the blood.

Urine Pregnancy Test
You would order this lab test for a female of child-bearing age.

Urinary 4: Difficulty Urinating or Can NOT Urinate

Urinary Problem, Possible Kidney Stone (Urinary 3)
You would make this CHAM Assessment for a patient presenting with blood in urine, and has history of blood in urine and passing "gravel", intermittent back pain on one side that radiates to groin, no fever.

Ureters

You would order these lab tests if a male presents with discharge or pain with urinating.

Urinary 4: Enlarged Prostate
The following is Patient Education for which urinary Assessment: "[This problem] can cause difficulty starting urine stream. A weak urine stream. Difficulty emptying bladder completely. The need to urinate more often than usual. Need to get up more often at night to urinate."

Kidney Infection (Urinary 2)
You would make this CHAM Assessment for a patient presenting with fever, back pain on one side, nausea and vomiting, and pain with urinating.

Dehydration, certain diets, or may run in families.
These are the causes of kidney stones. The answer can be found in the patient education section of the CHAM plan for this problem.

Urethra
This muscular tube drains urine from bladder to outside the body.

Because of possible antibiotic resistance your doctor may instruct you to perform this Additional Care for Urinary 1: Bladder Infection

Urinary Catheter to Drain Urine
A doctor my instruct you to perform this Special Care procedure for a patient with Difficulty Urinating.

Can Not Control Urination (Urinary 5)
You would make this CHAM Assessment for a patient presenting with no urinary pain or fever. Urine leaks when coughing, laughing, or sneezing.

Is 18 years or younger.
Is male. Is female with vaginal discharge on exam. Is pregnant. Has blood in urine and NO pain. Has had 3 or more urinary tract infections in the past 12 months. Has any other High Risk Health Condition.
Even if you have a Standing Order for a Bladder Infection you still must report the patient if they meet any of these 7 criteria.
