There is more than one item number listed on the RMP bill, the utility bill is over a month old, the utility bill is a Final Notice or there is a 48-hour notice, when a client reports they are all-electric, when no bill is provided, when the account number is unclear or incomplete on the bill.

A birth certificate, Medicaid card, immigration document, immunization card, blessing certificate, hospital certificate, government ID with date of birth

If the utility is under the landlord's name, utilities are included in the rent or utilities are on a Master Meter.

Client's date of birth on the application.

A Landlord Statement, Utility Verification with the utility company, printout showing usage for gas or electricity (i.e. Relms, Conservice, etc.)

A handicapped placard, SSI (Supplemental) for someone under 60 years, Medicare for someone under 60 years, Veteran's Benefits that list disability, "ABD" marking from DWS (Aged Blind Disabled), "DI" or "HA" marking on social security benefit letter, Medical Disability Form from MD or PA, visible disability.

The client must show that they have a bill in their name or pay a monthly usage for utilities.

Most recent tax return form, Self-Employment Form AND Tax Authorization Form.

The HEAT benefit is not enough to guarantee service, client's account is from a previous address, client's account at their current address has gone "legal" or can't be pulled back from collections, client is on subsidized housing and their utilities are included in the rent.

Social security card, Medicare card (if number is listed), Social Security benefit letter (if full number is listed), W-2, eShare verification, printout from DWS with the full number listed, Permanent File from previous HEAT season.
