What is Contact/Making contact?
You would use this qualifier/sub-qualifier when you attempt to reach a patient/family for the first time after receiving a referral.

What is Appointment/Treatment
You would use this qualifier/Sub-Qualifier when a non-admit report has been submitted by a nurse because program management is determining if they can admit the patient while they are receiving IV Antibiotics.

What are VITAS Volunteers/Paw Pals?
These volunteers bring pets to residential facilities and IPUs to visit patients.

What is Appointment/Scheduled?
You would use this qualifier/sub-qualifier when an appointment has been scheduled and you are assigning a nurse to submit their report.

What is Contact/Referral Source
You would use this qualifier/sub-qualifier when you've spoken to the caregiver and they request you call the social worker who referred the patient the next morning when she is back to request a hospice referral. An admissions resource has not me with the patient or family yet.

This service involves clinically trained individuals who play music for patients to alleviate psychological and physical symptoms.

You would use this qualifier/sub-qualifier when an admission resource has met with a patient and family and submitted a Non-Admit report stating the patient must be assessed for charity care by a Caseworker because they have no insurance.

You would use this qualifier/sub-qualifier when an admission resource met with a patient/family member over the phone and you are assigning a nurse to complete an evaluation.

What is Contact/Direction
You would use this qualifier/sub-qualifier when you have made several attempts to reach the patient and family unsuccessfully, an admission resource did a wellness-check at the patient's home and no one answered, and the Referral Source was the patient's daughter who is also not returning your calls.

What is a Traveling Patient?
*Please note patients traveling from one VITAS program to another will not involve admissions unless they are permanently transferring.
This type of patient is on service with another hospice and stays in a VITAS program territory for a limited amount of time. They need a special contract which is coordinated by your Business Office Manager.

You would use this qualifier/sub-qualifier when a non-admit report has been submitted stating hospital staff are awaiting acceptance at a LTC facility before a discharge order can be obtained.

What is Admit/Transfer to Patient Care
You should use this qualifier/sub-qualifier when an admitted at time of visit report was submitted by an admission nurse, the patient's chart has been validated, and all necessary information is entered to activate the patient's chart for the care team

This service provides up to five days at a time of care at a nursing facility to provide a break to caregivers at home.
