Complete and fill out the Incident Report form. The form is for either severe or minor incidents, property damage, theft, or other losses, including motor vehicle accidents. The form should also be used to report injuries to non-employees (i.e.: volunteers, youth members, visitors) Please submit the information within 48 hours, you can amend the report later if more information becomes available.
What form does a project leader need to fill out if an incident occurs during a 4-H activity? How quickly would you need to submit the report?

The signatures must be from two (2) unrelated individuals, of whom one can be a 4-H member and the other an adult volunteer. The signer may not be the same person as the payee. If the bank does not allow youth members to sign, then there should be two adult volunteers.
Who and how many check signers need to be present on a club check?

4-H adult volunteers who do not annually reapply to continue as a 4-H adult volunteer are considered to have a break in service. This means they will need to complete the "new volunteer" requirements.

1.Adults should not be alone in vehicles with members.
2.Adult volunteers should not be one-on-one in a vehicle with members other than their own child. 3.Adult volunteers must have current Member Treatment Authorization and Health History Form - English and Health History Form from parents/guardians before transporting members in any vehicle to any 4-H YDP activity or event.

1. Set up a meeting with the parent and project leader and invite another leader in your club to attend as mediation. Try to resolve the disagreement at the club level.
2. If the issue is still unresolved either party can choose to fill out the formal 4-H Complaint form. This can then be turned in to the 4-H office for further guidance.
There is a disagreement between a project leader and a parent. This conversation takes a turn for the worst and a parent storms off angry and upset. What would be the appropriate steps to resolve this disagreement between leader and parent?

Funds need to be deposited in club account. Project leader needs to submit receipts for reimbursement of purchased project materials. Project leader needs to notify CCL/Treasurer to insure funds are reflective in club budget.
